AgeRight Clinical Services (ACS) was created to meet the ever-increasing medical needs of older adults and geriatrics. Created by long-term care providers for long-term care providers, ACS is the first and only clinical group of its kind. As a clinician with AgeRight Clinical Services, you will work on a team of dedicated Physicians, APPs, Nurse Practitioners, and Physician Associates from a variety of different specialty backgrounds to deliver compassionate, holistic care to our patients. This is a fantastic opportunity for an experienced Family, Adult, or Geriatric Nurse Practitioner or Physician Associate who desires to work in a clinic setting within a senior living community, that enjoys autonomy, and is looking to exercise their clinical skillset in our ever-evolving environment. You will have the opportunity to build relationships with your patients and make a meaningful difference in their lives.

Requirements

Current, unrestricted Oregon state license, or ability to obtain an Oregon state NP or PA license and current board certification in your field
Masters Degree in Nursing or Physician Assistant Studies
Proficiency in medical documentation and coding
Current DEA certificate

Nice To Haves

Experience working with older adults in a clinic, medical office, or urgent care setting preferred

Responsibilities

Conduct clinical interventional and episodic direct care visits, and appropriate re-evaluation of care visits.
Diagnose and treat common acute illness and injuries
Order and interpret diagnostic tests
Develop and implement treatment plans
Collaborate with physicians and other healthcare professionals to ensure coordinated patient care
Perform minor surgical procedures as needed (repair lacerations, I & D, etc.)
Review medications, orders, and diagnostic tests, and respond to clinical abnormalities appropriately for all patients under their care, as allowed and/or requested by collaborating physicians.
Engage in advanced care planning conversations
Conduct Annual Wellness Visits on all senior living residents, as applicable
Coordinate care with other disciplines, such as Therapists, Specialists, and Social Workers
