About The Position

We are seeking a Nurse Practitioner or Physician Assistant to provide direct in-person care at skilled nursing and post-acute care facilities across Mesa, Gilbert, and Chandler, AZ. This role is ideal for a provider seeking flexibility and autonomy while making a meaningful impact on patient outcomes in long-term care settings. As an independent contractor, you will play a vital role in maintaining continuity of care and reducing unnecessary hospitalizations, ensuring patients receive high-quality treatment throughout the weekend.

Requirements

  • Active, unrestricted Arizona license as a Nurse Practitioner or Physician Assistant
  • Graduate of an accredited U.S.-based NP or PA program (Puerto Rico-based programs not eligible)

Nice To Haves

  • 1–2 years of experience in post-acute care, long-term care, or skilled nursing facilities
  • Proficiency with EMR systems and post-acute documentation standards
  • Excellent clinical judgment, communication, and organizational skills
  • Ability to work independently within clearly defined clinical parameters

Responsibilities

  • Complete weekend rounds across multiple skilled nursing facilities
  • Perform patient evaluations and develop individualized care plans
  • Manage acute conditions and chronic disease exacerbations
  • Order, review, and act on laboratory, imaging, and diagnostic results
  • Adjust medications and therapies as clinically appropriate
  • Collaborate closely with facility staff, physicians, and interdisciplinary teams
  • Educate patients, families, and facility staff regarding treatment goals and expectations
  • Document all clinical encounters promptly and accurately in the EMR
  • Support discharge planning and smooth care transitions
  • Ensure compliance with SNF protocols and regulatory requirements
