About The Position

The General Surgery and Surgical subspecialty Advanced Practice Nurse (APN) or Physician Assistant utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. The General surgery and surgical subspecialty Nurse Practitioner or Physician Assistant manages the overnight inpatient and operative needs of surgical patients in conjunction with, and under the supervision of an attending physician . The NP is responsible for the assessment and management of various patient populations which includes the overall management of the orthopedic surgical patient, ordering and interpretation of diagnostic testing, development of appropriate plans of care, therapeutic interventions. They maintain a collaborative work environment that is safe, just, and founded on principles of high reliability and quality outcomes for patients and colleagues. The NP will perform duties in accordance with state and federal regulations as well as network policies, NP scope of practice guidelines, approved clinical privileges and the JFKUMC NP policy. The Nurse Practitioner reports to the department clinical lead and Director of Advanced Practice Providers, Full time includes weekends and holidays on a rotational basis. Minimum of 2 years experience in the Nurse Practitioner / Physician Assistant role preferred Minimum of 5 years RN/Specialty experience. RNFA certification.

Requirements

Graduate of an NLN/AACN accredited nursing program.
Master's or Doctorate degree from an accredited Nurse Practitioner program.
Maintains clinical competencies through continuing education and clinical practice.
Experience in a relevant patient care setting (or as regulations dictate).
National Provider Identifier (NPI).
Prescriptive authority through CDS and DEA registration according to New Jersey state law.
Collaborative agreement with physicians on staff.
Excellent written and verbal communication skills.
Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
NJ State Professional Registered Nurse License.
Advanced Nurse Practitioner (w/ Master's Degree).
BLS AHA Basic Health Care Life Support HCP Certification.
Board certified by AANP or ANCC.
RNFA
Successful completion of an approved physician's assistant program, accredited by the AMA Council on Medical Education.
Excellent written and verbal communication skills.
Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms
Physician Assistant License.
National Commission on Certification of Physician Assistants.
BLS AHA Basic Health Care Life Support HCP Certification.

Responsibilities

Obtains a thorough patient history that includes: Determination of their health status and primary care service needs. Analysis of primary symptoms. Assess patient compliance with historical primary care treatment plan if one exists. Complete medical and social history. Performs an appropriate physical examination and recognizes normal and abnormal findings. Records findings of the physical exam in a clear and concise manner using the appropriate format. Connect patient with a primary care physician based on the data gathered in the history and physical examination. Counsel, educate, and instruct patients in specific disease-related and preventative medicine areas.
Communicates with assigned Primary Care physician regarding pertinent patient information including but not limited to actual or potential clinical problems. Collaborates with physicians, family members, social workers, community contacts and other members of the health care team in patient care planning.
Utilizes assessment skills and collaborates with other disciplines to develop a comprehensive education plan to meet patient/family needs. Utilizes assessment skills and collaborates with other disciplines to develop a comprehensive plan of care that meets patient needs. Incorporates diagnosis, interventions, and expected outcomes. Communicates plans of care with the patient and family. Identifies the patients educational needs based on the assessment of the patient.
Understands their role as a professional and conduct themselves in accordance with the same. Seeks out other professionals to network and create positive alliances. Is committed to a positive meaningful interaction with the patients. Will oversee Community Outreach workers, and conduct weekly meetings to discuss patient progress, goals and issues/concerns.

Orders, performs, and interprets laboratory tests and diagnostic studies/procedures in collaboration with physicians and consultants as appropriate.
Manages general medical and surgical conditions based on comprehensive knowledge of etiologies, risk factors, pathophysiology, and the indications and contraindications of pharmacologic agents and other treatment modalities.
Prescribes medication safely and effectively in accordance with best practices, hospital policy, and NJ regulations, providing patient education as needed.
Seeks opportunities to promote Hackensack Meridian Health (HMH) APN practice recognition (locally, nationally, and internationally) through conferences, publications, and presentations.
Provides care that is patient- and family-centered, compassionate, and effective for the promotion of health and treatment of disease.
Practices as a healthcare provider according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges.
Adheres to HMH nursing protocols, standards of care, and disease-specific quality indicators.
Applies evidence-based and value-based medicine to systematically analyze and improve patient care practices and outcomes.
Evaluates patient/family transition of care planning, considering resource utilization, cost, quality, safety and self-management promotion for chronic conditions.
Demonstrates clinical expertise through advanced assessment and procedural skills pertinent to the practice specialty.
Completes medication reconciliations as appropriate.
Participate in annual APN peer review.
Translates `best practice' into clinical practice by evaluating current protocols and standards of care.
Provides culturally sensitive and educationally appropriate health management education to patients and families.
Facilitates consistent, coordinated care and clear communication among all members of the healthcare team through participation in daily interdisciplinary rounds.
Serves as an advocate for patients and families, identifying and addressing obstacles to care progression and escalating issues as appropriate.
Serves as a clinical resource for the multidisciplinary team by addressing clinical, process, and system improvement opportunities.
Participates in organizational and departmental meetings, committees, and workgroups as necessary.
Demonstrates the highest level of accountability for professional practice and adheres to ethical principles regarding patient confidentiality, informed consent, and adverse outcomes.
Collaborates with medical staff to support nursing-led protocols.
