JOB PURPOSE: Provide direct primary care to patients, with emphasis on geriatric care, including treatments and medications. Locations of care include clinic, nursing home, hospice, assisted living and patients' homes.

Requirements

  • MINIMUM QUALIFICATIONS – Nurse Practitioner Education: Master’s degree in Nursing Experience: 1-3 years nurse practitioner experience preferred Required licensure, certification, registration, or other requirements: Valid State of Wyoming authorization as a Nurse Practitioner.
  • Prescriptive authority in the State of Wyoming.
  • National certification in degree specialty.
  • For certain specialized positions, a DEA registry license may be required.
  • Valid CPR certification (cardiopulmonary resuscitation)
  • Valid BLS certification (basic life support)
  • MINIMUM QUALIFICATIONS – Physician’s Assistant Education: Master’s degree Experience: 3 years as a Physician’s Assistant Required licensure, certification, registration or other requirements: National and valid State of Wyoming certification as a Physician’s Assistant
  • Prescriptive authority in the State of Wyoming
  • Licensure by the Wyoming Board of Medicine
  • The level of this position will be determined based on the selected candidate’s experience.

Responsibilities

  • Conduct comprehensive health assessments and physical examinations for patients.
  • Complete recertifications as required by Medicare.
  • Develop and implement individualized, long-term care plans.
  • Manage chronic diseases and address acute medical issues.
  • Prescribe medical care to patients, including treatments and medications.
  • Collaborate with multidisciplinary teams to coordinate patient care.
  • Monitor patient progress and adjust care plans as needed.
