About The Position

Provide direct primary care to patients, with emphasis on geriatric care, including treatments and medications. Locations of care include clinic, nursing home, hospice, assisted living and patients' homes.

Requirements

  • Master’s degree in Nursing (Nurse Practitioner)
  • 1-3 years nurse practitioner experience preferred (Nurse Practitioner)
  • Valid State of Wyoming authorization as a Nurse Practitioner (Nurse Practitioner)
  • Prescriptive authority in the State of Wyoming (Nurse Practitioner)
  • National certification in degree specialty (Nurse Practitioner)
  • For certain specialized positions, a DEA registry license may be required (Nurse Practitioner)
  • Valid CPR certification (cardiopulmonary resuscitation) (Nurse Practitioner)
  • Valid BLS certification (basic life support) (Nurse Practitioner)
  • Master’s degree (Physician’s Assistant)
  • 3 years as a Physician’s Assistant (Physician’s Assistant)
  • National and valid State of Wyoming certification as a Physician’s Assistant (Physician’s Assistant)
  • Prescriptive authority in the State of Wyoming (Physician’s Assistant)
  • Licensure by the Wyoming Board of Medicine (Physician’s Assistant)

Nice To Haves

  • 3 Years of Geriatric Experience
  • Certification or Focused Training in Geriatric Care
  • Experience in Providing Geriatric Care
  • Experience Working in Assisted Living Facilities and/or Skilled Nursing Facilities
  • FQHC Experience
  • Healthcare Student Teaching Experience
  • PCMH Experience

Responsibilities

  • Conduct comprehensive health assessments and physical examinations for patients.
  • Complete recertifications as required by Medicare.
  • Develop and implement individualized, long-term care plans.
  • Manage chronic diseases and address acute medical issues.
  • Prescribe medical care to patients, including treatments and medications.
  • Collaborate with multidisciplinary teams to coordinate patient care.
  • Monitor patient progress and adjust care plans as needed.
