Requirements

  • Must have at least one (1) year of primary care experience
  • Valid Licensure as a Nurse Practitioner or Physician Assistant in State of Practice
  • Ability & Desire to work independently, and drive between communities.
  • We currently use Elation as our EMR
  • Effective verbal & written communication & time management skills
  • Valid Fingerprint Clearance Card
  • Current CPR Certification & Proof of Negative TB Test

Responsibilities

  • Delivering mobile on-site primary care services to our vulnerable patient population residing in assisted living and other senior communities.
  • Autonomous management of complex healthcare scenarios, including multiple co-morbid conditions.
  • Educate patients and their families on self-care, disease management, and end-of-life care.
  • Collaboration with facility staff and Atlas Patient Care support teams.
  • Upholding confidentiality in all job aspects
