The Nurse Practitioner / Physician Assistant is responsible for delivering comprehensive primary care, palliative care, and chronic condition management to a dedicated panel of geriatric patients in place. This includes visiting private homes, apartment buildings, and long-term care settings such as assisted and independent living facilities. The Nurse Practitioner / Physician Assistant provides care to medically complex, frail, and geriatric adults who may have one or more of the following situations: multi-morbidity, polypharmacy, cognitive impairment, functional decline, and significant caregiver burden. Patients typically have mobility limitations and experience high clinical complexity, evolving goals of care, and increased risk for avoidable utilization. Providers are expected to build and maintain therapeutic relationships with patients and caregivers, including providing emotional support and education. Providers see an average of six (6) patients per day. Based on patient need, more visits may be scheduled with provider approval and are reimbursed additional to the hourly rate. The practice uses AthenaCare EHR.
