The Nurse Practitioner or Physician Assistant is responsible for delivering comprehensive primary care to a dedicated panel of geriatric patients. This includes visiting assisted and independent living facilities (ALFs / ILFs), private homes (house calls), and other group settings (apartment buildings, senior housing complexes, etc.). Providers see an average of six (6) patients per day. Based on patient need, more visits may be scheduled (if work hours permit) and are reimbursed in addition to the hourly rate. The new provider will see medically complex, frail, and geriatric adults who may have: - high clinical complexity - multi-morbidity - functional decline - mobility limitations - polypharmacy - cognitive impairment - caregiver burden - increased risk for avoidable utilization - evolving goals of care This requires providers to build and maintain therapeutic relationships with patients and caregivers, including providing emotional support and education as needed. The practice uses the AthenaCare EHR.
