The Nurse Practitioner - Endocrinology Clinic is responsible for providing specialized clinical care to patients with diabetes and related metabolic illnesses. The role centers on assessment, diagnosis, treatment, education, and continuous management of individuals with Type 1, Type 2, and gestational diabetes, as well as prediabetes, obesity, and connected comorbidities. The NP will work in partnership with physicians and multidisciplinary teams to improve glycemic control, reduce complications, and advance patient outcomes using evidence-supported practice and patient-focused care. Opportunities to contribute to general endocrinology patient care are available should the NP be interested.

Requirements

  • Master of Nursing Degree.
  • Graduate of an accredited Nurse practitioner program approved by the California Board of Registered.
  • One (1) plus years’ experience as Nurse Practitioner with a background in endocrinology or diabetes treatment.
  • CA RN License.
  • Nurse Practitioner and Furnishing License.
  • National Certification or eligibility for certification as a Nurse Practitioner.
  • DEA License (schedule 2-5).
  • Basic Life Support Certification

Nice To Haves

  • Experience managing diabetes devices such as continuous glucose monitors (CGMs) and insulin pumps highly preferred.

Responsibilities

  • Perform comprehensive patient assessments, including medical history, physical examination, and risk evaluation.
  • Diagnose and manage diabetes and related metabolic disorders.
  • Develop and implement personalized care plans and individualized treatment plans, including medication management and lifestyle interventions.
  • Implement and evaluate treatment plans, adjusting as necessary to achieve efficient patient outcomes.
  • Prescribe and adjust medications, including insulin and non-insulin therapies, in accordance with state regulations and within scope of practice.
  • Monitor and interpret laboratory results (e.g., A1C, glucose levels, lipid profiles) to track patient progress.
  • Manage acute and chronic complications of diabetes (e.g., neuropathy, nephropathy, cardiovascular risk).
  • Provide education and counseling to patients and their families on disease management, medication adherence, lifestyle modifications, and diabetes self-management.
  • Assist with continuous glucose monitoring (CGM) and insulin pump management when applicable and within scope of practice.
  • Conduct follow-up visits to evaluate treatment effectiveness and patient adherence.
  • Collaborate with endocrinologists, physicians, dietitians, diabetes educators, pharmacists, and other healthcare professionals to deliver comprehensive care.
  • Coordinate referrals to specialists as needed (e.g., ophthalmology, podiatry, cardiology).
  • Participate in multidisciplinary case reviews, care planning, and quality improvement initiatives to enhance patient care and clinical practices.
  • Assist in prior authorizations, denial letters, and conduct 1:1 calls with insurance as needed.
  • Ensure continuity of care across outpatient and inpatient settings when applicable.
  • Stay current with the latest advancements in diabetes care and endocrinology.
