Nurse Practitioner (Neurology - Memory Care) - Ahwatukee, AZ
About The Position
This is a dedicated outpatient neurology role in Ahwatukee, AZ, centered on memory care and movement disorders. The Nurse Practitioner will work closely with adult patients experiencing conditions such as Alzheimer’s disease, Parkinson’s disease, and other cognitive or neurologic impairments. The schedule is Monday – Friday, 8:00 AM – 5:00 PM. The role involves evaluating patients, managing ongoing treatment plans, and providing continuity of care in a setting that allows for deeper clinical relationships. This position offers a balance of autonomy and collaboration, working alongside neurologists and a supportive clinical team while maintaining ownership of your patient panel. Expect a steady clinic flow with a focus on thoughtful assessments, medication management, and patient/family education. Requirements
- Active Nurse Practitioner license (or ability to obtain in Arizona)
- Board certification as an NP
- Experience in neurology, geriatrics, internal medicine, or a related field
- DEA license (or ability to obtain)

- Exposure to movement disorders (e.g., Parkinson’s disease)
- Experience in an outpatient specialty clinic setting
- Comfort with chronic disease management in adult populations
Responsibilities
- Conduct comprehensive neurological evaluations and patient assessments
- Diagnose and manage memory-related and movement disorders
- Develop and implement individualized treatment plans
- Prescribe and manage medications as appropriate
- Provide patient and family education on disease progression and care plans
- Monitor patient progress and adjust treatments as needed
- Collaborate with neurologists and interdisciplinary care teams
- Maintain accurate and timely documentation in the EMR
- Coordinate care with specialists and ancillary services when needed
- Support continuity of care through follow-ups and long-term management
