About The Position

Sutter West Bay Medical Group (SWBMG) is seeking a Nurse Practitioner to join our Kidney Transplant Program in San Francisco. This is a full-time outpatient opportunity to provide comprehensive care and long-term follow-up for kidney transplant recipients, donors, and candidates in collaboration with a multidisciplinary transplant team. You will join a highly experienced and collaborative group dedicated to improving transplant outcomes through evidence-based practice, patient education, and compassionate, team-based care. Position Details Outpatient role with no inpatient or call responsibilities Manage care for kidney transplant recipients, including immunosuppressive management, follow-up care, and coordination of specialty services Conduct pre-transplant evaluations and assist with patient education for recipients and living donors Collaborate with transplant surgeons, nephrologists, pharmacists, dietitians, and social workers to ensure integrated, patient-centered care Participate in quality improvement initiatives and program development Maintain timely and accurate documentation within the electronic health record

Requirements

  • Current California Registered Nurse (RN) and Nurse Practitioner (NP) license
  • Master's degree in Nursing from an accredited program
  • National NP certification (ANCC or AANP)
  • Current DEA, BLS, and ACLS certification

Nice To Haves

  • Minimum of 2 years of experience as an NP in transplant, nephrology, internal medicine, or surgical specialty
  • Prior experience managing kidney transplant or solid organ transplant patients highly desired
  • Strong communication, collaboration, and critical thinking skills
  • Commitment to patient education, long-term wellness, and multidisciplinary teamwork

Responsibilities
