About The Position

Lifestream Health Center, a leading provider of pain management and mental health services, is seeking a Nurse Practitioner to join our growing team in our Baltimore satellite clinics (Woodlawn, Fells Point, Parkville, Glen Burnie). As a Nurse Practitioner in one of our satellite offices, you will play a crucial role in delivering high-quality, whole-person care to patients experiencing chronic pain, depression, or substance use disorders. You will manage your clinic with professionalism and autonomy, while benefiting from strong support and coordination with our main office. Clinic hours for this role are Monday - Thursday.
Requirements
- Current and unrestricted Nurse Practitioner license in Maryland.
- Active CDS and DEA licenses.
- Demonstrated strong clinical judgment, excellent time management, and meticulous follow-through.
- A patient-first mindset, committed to providing compassionate and comprehensive care.
- Comfort and proficiency working independently in a satellite clinic setting, with full remote support.
Nice To Haves
- Experience in pain management, mental health, behavioral health, or addiction medicine is preferred.
Responsibilities
- Perform thorough patient assessments, formulate accurate diagnoses, and develop individualized treatment plans.
- Manage same-day patient visits and ensure all documentation is completed accurately and timely (within 72 hours).
- Provide in-house treatments such as trigger point injections and pharmacy refills.
- Facilitate appropriate referrals for imaging services, Physical Therapy, Chiropractic care, Orthopedic consultations, and laboratory services.
- Coordinate care with the main office to ensure seamless patient transitions and support.
