NURSE PRACTITIONER FELLOW This vacancy will be posted on internal Career Website for 10 work days. Associates are eligible to apply for a transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. About the Montefiore Nurse Practitioner Residency Program: Montefiore Family Nurse Practitioner Fellowship Program provides new nurse practitioners committed to practice careers as PCPs with an intensive training experience focused on training to clinical complexity and high performance. The NP Residency Program provides a highly structured transition from academic preparation to practice that supports the development of confidence, competence, and mastery in the primary care and FQHC setting. This 12-month, full-time position is a unique employment and training opportunity for new NPs to start their practice career in a high-performance health system in a supported learning environment Upon completion of the program, the NP Fellow will be required to commit to a 2-year contract to work at a Montefiore Medical Group health center site. Key Responsibilities: Develop and manage a panel of patients with the exclusive attention of an expert primary care provider. Perform a comprehensive, evidence-based assessment. Use advanced clinical judgment to diagnose. Synthesize relevant data to develop a person-centered, evidence-based plan of care. Plan and manage care for individuals and populations across the continuum including prescribing, ordering, and evaluation of therapeutic interventions. Educate individuals, families, and communities to empower them to participate in their care and enable shared decision-making. Works as part of an integrated primary care team focusing on diversity of chief complaints, efficiency, episodic and acute care Develop a person-centered, evidence-based plan of care tailored to each patient's unique needs. Use advanced clinical judgment to diagnose complex health conditions and provide appropriate care. Synthesize clinical data to create detailed care plans that include prescribing, ordering, and evaluating therapeutic interventions. Prescribe medications and manage prescriptions based on evidence-based guidelines. Participate in weekly didactic sessions focusing on high-volume, complex, high-risk, and commonly seen topics in primary care. Assigned Sites: The NP Fellow will be assigned to one of two primary care sites: MMG Via Verde Site 1825 Boston Road Site Program Structure: The Nurse Practitioner Fellowship Program is a 5-day-a-week commitment, with 4 days in clinical practice and 1 day dedicated to classroom/didactic learning. This structure ensures a balanced approach to developing hands-on clinical skills alongside essential academic and didactic education.

Requirements

  • FNP Degree: Must have received a Nurse Practitioner (NP) degree in Family Medicine within the last 48 months.
  • Licensing: Must hold a current and active New York State Nurse Practitioner (NP) license and registration.
  • Certification: Must be board-certified as a Nurse Practitioner, or eligible for certification.
  • Experience: Open to newly licensed Nurse Practitioners or those with less than one year of clinical experience in family medicine.
