About The Position
Nurse Practitioner – Family Practice JOB OVERVIEW • Job Title: Nurse Practitioner – Family Practice • Job Type: Locum Tenens • Location: OR (Multiple Locations) • Service Setting: Inpatient/Outpatient • Coverage Type: Clinical Only • Coverage Period: April 13, 2026 – Ongoing (Month-to-Month) • Clinical Shift Schedule: Monday – Friday, 8:00 AM – 5:00 PM COVERAGE DATES • Starting April 13, 2026 or as soon as credentialed • Month-to-month assignment with option to extend • Full-time coverage, 40 hours per week PATIENT INFORMATION • Patient Demographics: Pediatrics and Adults • Patient Volume/Census: 12–14 telehealth visits per day; 6–7 home visits per day; ~10 mixed • Phone Consults: Yes • Case Mix: Annual risk assessments and chronic condition reviews FACILITY INFORMATION • EMR System: Salesforce (availability tracking) • Office Equipment Available: Equipment provided by client • Specialty Backup Available: Primary Care Providers • Reason for Coverage: Supplemental coverage COMPENSATION & BENEFITS • Travel, Lodging, and Malpractice Insurance: Covered per client policy REQUIRED PROCEDURES • Annual risk assessments • Chronic condition reviews • Telehealth evaluations • In-home patient assessments JOB REQUIREMENTS • Licenses: Active OR license required • Board: Board Certified in Family Practice • Experience: Experience with telehealth and home health preferred • Other Qualifications: Must be enrolled/credentialed in Medicard; comfortable seeing pediatric and adult patients DUTIES & RESPONSIBILITIES • Conduct telehealth and in-home patient assessments • Perform annual risk assessments and chronic condition reviews • Document findings and maintain accurate patient records • Escalate urgent findings to the patient’s primary care provider • Maintain availability scheduling to ensure full-day compensation eligibility
Requirements
- Active OR license required
- Board Certified in Family Practice
- Experience with telehealth and home health preferred
- Must be enrolled/credentialed in Medicard
- Comfortable seeing pediatric and adult patients
