About The Position

The Nurse Navigator (NN) acts as a multidisciplinary team leader and liaison between patients, family, physicians, clinical care teams, community organizations and BHSF leadership. The NN is responsible for leading coordination of care across the continuum, providing education, support and resources to patient/family, facilitate shared decision making, patient advocacy, ensuring timely access to quality multidisciplinary care and patient satisfaction. The NN will monitor individuals, populations and program outcomes and engage in quality improvement activities. Estimated salary range for this position is $86465.60 - $114999.25 / year depending on experience.

Requirements

  • Bachelor Science Nursing - BSN
  • Advanced Cardiovascular Life Support
  • Basic Life Support
  • Neonatal Resuscitation Program
  • Pediatric Advanced Life Support
  • Registered Nurse
  • BLS required
  • Must have an active Registered Nurse license
  • Certification in area of expertise required
  • Experience working in a hospital, managed care or medical office preferred
  • Five years nursing experience in key relevant areas strongly preferred: MedSurg, Oncology Care (Inpatient/Outpatient Oncology, Infusion, Radiation, BMT, etc.), Patient Navigation or related service (Case Management, Care Coordination, Social Work, Population Health, etc.), or specific Disease Sites (Breast, GI, GU, Neuro, etc.)
  • Skilled in Microsoft Office, EMR applications (Word, Excel, Sharepoint, TEAMS, HEDIS, STAR, etc.) and typing
  • Life Support Certificate (BLS, NRP, PALS, ACLS) as per department standard

Nice To Haves

  • Bilingual English/Spanish preferred
  • Experience working in a hospital, managed care or medical office preferred
