About The Position

A Day in the Life Act as a liaison between patients, professional staff and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs and educational needs. Serves in an expanded health care role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality cand cost-effective health care services. Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization. Support quality improvement initiatives and adherence to safety standards.

Requirements

Licensed RN in the State of Michigan
Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella.
MHC also requires all employees to receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.

Nice To Haves

Experience in complex chronic disease management preferred
Bachelor of Science degree in nursing preferred
Five years' (5) experience as an RN preferred
