In collaboration with a multidisciplinary team, the Nurse Navigator – RN serves as an advocate for patients with breast health concerns, particularly those facing serious illnesses such as breast cancer. By providing care coordination, education, and emotional support to patients, families, and caregivers, the Nurse Navigator ensures a seamless care continuum from diagnosis through treatment and survivorship. This role is pivotal in ensuring that patients receive high-quality, personalized care across all care delivery sites. The Nurse Navigator is responsible for managing resources and providing a comprehensive support system, serving as a liaison between patients, families, and a wide range of healthcare providers. This includes internal and external specialists, oncologists, radiologists, surgeons, and mental health professionals, along with connecting patients to support networks within the community. With a focus on breast health, the Nurse Navigator plays a key role in educating patients about treatment options, recovery processes, and long-term care, all while helping to ensure that the patient’s needs and concerns are prioritized throughout the healthcare journey.
