About The Position

The Nurse Navigator functions as a member of the multidisciplinary team and serves as an advocate and educator for patients with all types of medical conditions. The Nurse Navigator's primary function is to support patients through complex healthcare environments for the optimization of care and outcomes. The Nurse Navigator facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex diseases, and streamline the intervention process. The Nurse Navigator guides patients and families through the healthcare system by assisting with access issues in an effort to expediate timeliness of care and treatment. Navigators achieve this goal by building relationships with patients and physicians, coordinating the defined plan of care, assisting with appointments, assisting to resolve barriers, providing education, and connecting with community resources. The navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for patients, including support with interdisciplinary and cross-facility coverage and collaboration. The navigator is responsible for timely data collection that promotes tracking of the navigation program performance in line with HCA Healthcare, division and facility goals (growth, quality, practice guidelines, etc). The navigator identifies opportunities to streamline care practice through evidence-based guidelines and ensures quality, patient-focused care.

Requirements

  • 1-3 years of clinical experience - Required
  • Basic Cardiac Life Support (preferred if remote position) - Required
  • Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation. - Required

Nice To Haves

  • 3+ years of experience in as a Nurse Navigator and/or patient educator in service line - Preferred
  • Service Line Specialty Certification - Preferred
