Nurse Navigator, Lung & GI
About The Position
A professional registered nurse (RN) who offers individualized assistance to patients, families and caregivers to help overcome health care system barriers. This role enhances the patient experience and improves clinical outcomes by providing a seamless navigation process throughout the continuum of care. The Nurse Navigator collaborates with physicians and interdisciplinary teams to triage, coordinate, and manage patient care, educating patients and serving as a primary point of contact. The position also involves participating in strategic initiatives within the clinical program or service line, including developing, implementing, and monitoring navigation activities and sustainability. It facilitates shared decision-making and patient engagement by building trusting relationships and addressing patients' communication and health literacy needs, helping them integrate their values and preferences into treatment decisions. The ultimate goal is to promote positive outcomes for patients, providers, and the Sutter health care system.
Requirements
- Graduate of an accredited school of nursing
- RN-Registered Nurse of California
- BLS-Basic Life Support Healthcare Provider
- ONS Foundations of Oncology Nursing Practice within 90 days (if less than 1 year with oncology experience)
- 5 years recent relevant experience as an RN in an acute care facility

- Help patients integrate their values and preferences into treatment decisions
- Work for and promote positive outcomes for patients, providers and the Sutter health care system
