About The Position

This individual will manages the planning, development, implementation and oversight of health services. Provide support and consultation to other programs across agency. Manage a staff, work cross functionally with Physicians and NPs. Major Duties: Ensure participants residing at the facility receive services of highest quality and in compliance with agency, City, State, and Federal mandates. Identify health and mental health resources, focusing on preventive health services for our youth and families. Coordinate with providers for youth in agency to ensure high quality, Medicaid compliant services are provided. Provide/Arrange health and mental health education for staff, youth, and families. Collaborate with Staff at to ensure there is a multidisciplinary approach to care for youth. Responsible for staff and staffing needs. Monitor, audit and ensure medical and health home charts have appropriate documentation. Implement new Medicaid funded programming Update and/or develop any necessary procedural practices and provide training. Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma-sensitive culture. Perform other duties as assigned.

Requirements

  • Must be a graduate of an accredited nursing program, and possess a current New York License.
  • Current American Heart/or American Red Cross Certification in infant, child, and adult.
  • Team Player, motivates others, understanding of importance of compliance.
  • Demonstrate nursing principles
  • Ability to function as an educator and counselor to client, family and staff.
  • Adept in communication skills, both written and verbal.
  • Must have a commitment to work from a strength based and/or youth development trauma informed perspective.
  • Familiar with and comfortable with electronic health records, Microsoft Office Suite.
  • Full Time Monday -Friday 9am -5pm.
  • 2-5 years relevant experience is ideal.

Nice To Haves

  • Bachelor's degree preferred.
  • Successful completion of New York State HIV Counselor Training preferred.
