About The Position

The Integrated Veteran Care (IVC) Registered Nurse streamlines receiving, triaging, and directing timely, coordinated care. The nurse applies nursing principles with growing critical thinking, and has strong knowledge of referral processes, clinical review criteria, utilization management, documentation standards, community care regulations, and VHA guidelines. General Description of Assigned Duties: The Integrated Veteran Care (IVC) Registered Nurse (RN) supports the medical center's mission to improve timely access to care, empower Veterans to make informed decisions, and ensure eligible Veterans are appropriately referred and scheduled for community care when needed. Guides Veterans through the full spectrum of care options, including internal VA care and community-based services, using a national standardized triage tool. Serves as a liaison between referring VA providers and specialty care services by clinically triaging consults to confirm appropriate care requests and ensure completion of necessary pre-work and diagnostic testing. Complies with VA-mandated health information systems to store, exchange, and track referral care information. Triages Requests for Services (RFS) to promote access to in-house services when appropriate. Utilizes delegation of authority, when applicable, to function at the highest level of nursing practice and align care delivery with key performance indicators and business practice standards.

Requirements

  • Evidence of self-directed and independent nursing practice required.
  • Excellent interpersonal and communication skills are required.
  • Able to function autonomously and as an active member of an interdisciplinary team.
  • Demonstrates successful coordination of patient care on a multidisciplinary level required
  • 1 full and unrestricted license from any US State or territory

Nice To Haves

  • Minimum five years successful RN clinical experience in nursing with care/utilization management experience preferred.

Responsibilities

  • Triages incoming referrals and provides Veterans with care options and scheduling guidance.
  • Acts as a resource within the service for referral coordination and care navigation.
  • Supports safe, high-quality nursing care through knowledge sharing and collaboration with colleagues and other healthcare professionals.
  • Fosters a safe and supportive environment conducive to the professional development of healthcare staff.
  • Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
  • Responsible for documented outcomes at the program or service level, which must be broad, complex, and demonstrable at any organizational level within the facility.
