General Description of Assigned Duties: The Integrated Veteran Care (IVC) Registered Nurse (RN) supports the medical center's mission to improve timely access to care, empower Veterans to make informed decisions, and ensure eligible Veterans are appropriately referred and scheduled for community care when needed. Guides Veterans through the full spectrum of care options, including internal VA care and community-based services, using a national standardized triage tool. Serves as a liaison between referring VA providers and specialty care services by clinically triaging consults to confirm appropriate care requests and ensure completion of necessary pre-work and diagnostic testing. Complies with VA-mandated health information systems to store, exchange, and track referral care information. Triages Requests for Services (RFS) to promote access to in-house services when appropriate. Utilizes delegation of authority, when applicable, to function at the highest level of nursing practice and align care delivery with key performance indicators and business practice standards.
