The Physical and Nutritional Management Team (PNMT) Nurse is a dedicated registered nurse who, as part of PNMT, evaluates and analyzes assessment information and provides recommendations for individuals who meet PNMT referral criteria or for whom the interdisciplinary team requires assistance. Plans, develops, evaluates, and treats physical and nutritional management issues from the nursing perspective and scope of practice as part of PNMT. Collaborates with the Interdisciplinary Team (IDT) throughout the PNMT process to provide current assessments, test results, medical history, lab reports, and other information in order to optimize integration of PNMT recommendations and plans into the Individual Support Plan (ISP). Performs and reviews Nurse Post-Hospitalization Assessment/Review following hospitalization for conditions listed in the referral criteria for PNMT. Reports the results of the post-hospitalization assessments/reviews to the PNMT to help determine if a referral is needed for specialized PNMT interventions. Analyzes data and develops PNMT recommendations with measurable goals and action plans. Assists with treatment/interventions to individuals on active PNMT caseload and monitors individuals’ progress. Collaborates to ensure plans and supports are properly implemented. Reports findings and recommendations for interventions in assigned meetings such as PNMT, skin integrity committee, or other designated oversight groups. Submits required report, performs documentation, and contributes to policy development as assigned. Reviews literature and research findings to keep abreast of physical and nutritional management specialized topics, nursing care standards, state and federal laws (ICF/IID rules and regulations). As needed (such as due to staffing issues, complex cases, or for the purpose of on-the-job training), provides direct care nursing services. Provides treatment to HHS employees injured in course and scope of employment. Works under limited supervision, with considerable latitude for the use of initiative and independent judgment.
