About The Position

The Nurse Coordinator works in collaboration with the Assistant Regional Director and Residential Managers to coordinate and oversee healthcare services for program participants. This role ensures compliance with all medical, regulatory, and organizational standards while supporting quality assurance, staff training, and direct care needs. This is a hybrid position. The Nurse Coordinator demonstrates a strong commitment to RHD values, promoting high-quality care and supporting individuals’ health and well-being across assigned programs.

Requirements

  • Licensed Practical Nurse (LPN) required.
  • Valid driver’s license for three years and reliable transportation required.
  • Proficiency with Microsoft Office and electronic health record systems.

Nice To Haves

  • Minimum one year of experience coordinating healthcare services for individuals with intellectual or developmental disabilities (IDD) or similar populations preferred.
  • Knowledge of ODP licensing requirements and healthcare documentation standards preferred.
  • Strong organizational, written, and verbal communication skills.
  • Experience with medical record management and staff training.

Responsibilities

  • Conduct audits of medical documentation, including medical records and Electronic Health Records (EHR).
  • Utilize the Health Risk Screening Tool (HRST) to identify individuals with high medical needs and conduct periodic site visits.
  • Perform health assessments and provide triage when medical concerns arise.
  • Serve as the nurse of record for assigned programs and assist additional teams when needed.
  • Respond to after-hours medical calls and provide guidance to on-site staff.
  • Participate as a member of the interdisciplinary team, providing healthcare updates during case conferences and Individual Support Plan (ISP) meetings.
  • Accompany individuals and staff to medical appointments when necessary to help clarify provider instructions and educate staff on the purpose of the visit.
  • Coordinate with hospital case management teams during hospital admissions.
  • Maintain communication throughout hospitalization and participate in discharge planning meetings when appropriate.
  • Ensure all annual medical appointments and required follow-up documentation are completed according to regulatory and agency requirements.
  • Collaborate with leadership to ensure Durable Power of Attorney (DPOA) and Advance Directive documentation is current.
  • Accompany individuals to appointments as needed and ensure appropriate documentation and follow-up.
  • Develop and maintain a tracking system for medical appointments and follow-ups.
  • Communicate with insurance companies and external healthcare providers to advocate for individuals’ medical needs.
  • Review and upload Lifetime Medical Histories to the EHR annually and as needed.
  • Ensure Lifetime Medical Histories remain current and accurate.
  • Review Medication Administration Records (MARs) through the electronic MAR (eMAR) system.
  • Serve as liaison with the Incident Management team to review medication administration errors and coordinate training follow-up when necessary.
  • Coordinate with the Program Specialist regarding hospital discharges and medication changes.
  • Ensure required medication-related trainings are completed.
  • Communicate with prescribing providers and pharmacies to clarify or update medication orders.
  • Monitor and report medication errors and participate in quarterly medication audits.
  • Ensure compliance with medication storage, documentation, and administration standards.
  • Provide medical training for staff on topics such as: Diabetes care Seizure management Medication administration New medications or medical equipment Medical documentation requirements
  • Provide site-specific training during monthly staff meetings when needed.
  • Support Residential Managers in understanding medical documentation, procedures, and regulatory expectations.
  • Review HRST assessments for individuals identified with high medical acuity.
  • Participate in quality improvement initiatives focused on healthcare services and medication management.
  • Ensure compliance with internal policies and external regulatory requirements related to healthcare services and documentation.
  • Support data collection and analysis related to medication errors and clinical outcomes.
  • Maintain current healthcare releases and consent forms to ensure continuity of care.
  • Assist in developing, training, and implementing medical protocols that support participant health and safety.
  • Perform other duties as assigned.
