Job Description: Seeking a highly motivated nurse coordinator to join our multidisciplinary Chronic Obstructive Pulmonary Disease (COPD)/Asthma team within the Pulmonology clinic in the Canning Thoracic Institute. This nurse will join a strong and collaborative team where physicians, APPs (advanced practice providers) and a multi-disciplinary team work closely together to provide high quality direct patient care, consultation, and education to complex acute and chronically ill patients in the outpatient setting. The COPD/Asthma nurse clinician coordinator will also partner with medical and administrative leadership to prepare for program meetings, develop patient education materials, establish treatment protocols, and ensure operational efficiencies are met. This position is unique in that it allows the nurse to work autonomously and apply the full breadth of their critical thinking and decision-making skills. Organization, being a self-starter, working with a multi-disciplinary team and being detail-oriented are important in this role. The position will be hybrid; 5 8-hour shifts, with 2 remote. At least 1 year of clinical experience required.
