About The Position

Sacramento Post-Acute is a 5-Star 94-bed skilled nursing facility. We provide a range of services that include trach care, ventilator management, wound care, short-term rehabilitation, and daily living care for long-term patients. We currently have an opening for a full-time Case Manager Nurse. The schedule is Monday - Friday with flexible hours. Pay from $40-$45/hr. You will join an experienced team that consists of a Case Manager Assistant (handling transportation and re-auth insurance documentation), Social Services, IP Nurse, Rehab Director, Business Office, MDS, and Medical Records. Responsibilities

  • Manage HMO patients (non-Kaiser) from admission thru discharge.
  • Monitor patients' case throughout stay to ensure utilization of services and discharge planning are in accordance with plan of care.
  • Communicate with patient and/or family members.
  • Submit documentation for reauthorizations (initial authorizations are provided by admissions)
  • Collaborate with IDT to identify opportunities or barriers to patient care progression or a safe discharge.
