About The Position

The Nurse Case Manager coordinates and evaluates health care and social services for participants in the PACE program. Acting as a liaison between the interdisciplinary team (IDT), participants, caregivers, and community agencies, this role ensures seamless care delivery and support. Lead and collaborate with the IDT to develop and implement individualized care plans. Conduct assessments, home visits, and hospital discharge coordination. Maintain accurate documentation and communicate effectively with participants and families. Mentor staff, manage departmental operations, and participate in quality improvement initiatives. Ensure compliance with federal, state, and organizational regulations. Requirements

  • RN license in the state of the PACE organization
  • CPR certification required.
  • 2+ years of experience in geriatric care
  • 1+ year working with frail or elderly populations.
  • Valid driver’s license and insurance.
  • Strong clinical, leadership, and communication skills.

  • BSN preferred.
