About The Position

Veranda Rehab & Health Come join our team and start making a difference! As an RN/LVN for our organization, you will be responsible for the care of an amazing group of individuals! Shift: Monday- Friday 8:00a.m-5: 00p.m Full-Time Key Responsibilities Screening Planning Developing Organizing Implementing Monitoring Evaluating Record Documentation Skills assessment on residents Quarterly Nursing Reports Duties: • The Case Manager facilitates patient care by assessing patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress • Responsible for clinical and financial assessments of patients prior to admission • Facilitates and actively participates in interdisciplinary planning of clinical care of patients, and assists with outcome report completion • Provides outcome data to payers and Physicians on as needed basis • Serves as care liaison between patient, physician, care team members, and discharge planner by coordinating, monitoring, and communicating patient’s progress • Assists with coordination to most appropriate care level • Initiation and implementation of plan modifications as necessary through monitoring and re-evaluation to accommodate changes in treatment or progress • Communicates discharge information to physician and payer with outcome data, as appropriate Qualifications: • RN or LVN License is required • Case Manager experience is required • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public • Must be knowledgeable of nursing and medical practices and procedures EOE/Minorities/Females/Veteran/Disability Benefits: Medical, dental, vision 401K (Match) DailyPay Career advancement opportunities • Child Care Benefit Program • Scholarship Opportunities • Employee discounts on entertainment events, hotels, movies, theme parks, cell phones, and much more! Requirements

• RN or LVN License is required
