About The Position

The Caregiver Support Program (CSP) Coordinator Registered Nurse (RN) executes position responsibilities that demonstrate expert leadership, experience, and creative approaches in providing complex patient management. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Provides a state-of-the-art continuum of services, education, and resources to caregivers of Veterans with complex needs. Portland is a port city in the Pacific Northwest and the most populous city in the U.S. state of Oregon. Situated in the northwestern area of the state at the confluence of the Willamette and Columbia rivers. Portland is known for having the amenities of a major city (such as an international airport, an efficient public transit system, major league sports teams and many museums and art galleries) and the charms of a small city (such as plentiful arts and crafts fairs, independent bookstores and local traditions like the annual Rose Festival Parade). Rain or shine, you can take advantage of the city's proximity to nature by playing in the Willamette River, wandering through lush public parks and gardens, biking the city's 162 miles (261 km) of bike lanes and hiking trails both within city limits and in the nearby Columbia River Gorge.

Responsibilities

  • Applies advanced nursing clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating PCAFC and/or PGCSS eligibility.
  • Receives, reviews, and dispositions PCAFC and/or PGCSS initial eligibility decisions, reassessments, and appeal decisions for clinical eligibly determinations.
  • Evaluates current practice and formulates outcomes for groups of patients and/or organizational processes within area of expertise.
  • Works with interprofessional team members at facility and VISN levels to ensure support to caregivers is delivered objectively with strong, consistent documentation and communication in a direct and coordinated manner.
  • Provides and exhibits knowledge in assessment and monitoring, health restoration, wellness promotion, illness prevention, and coping facilitation.
  • Documents information utilized in clinical eligibility determination and eligibility assessment tools.
  • Assists the facility with MISSION Act implementation, as it relates to CSP.
  • Advocates for holistic care through patient/caregiver education and provider interaction and implements an educational plan to meet the patient care needs.
  • Uses clinical judgment in the interpretation of trends and patterns, application of regulation, policy, guidelines, and the recommendation of program enhancements.
