About The Position

The Nurse Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family. The Nurse Care Manager facilitates the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. In addition, the Nurse Care Manager helps drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements and patient satisfaction). The Nurse Care Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of service, promote efficient utilization of resources and ensure a well organized and safe plan of care is established for every patient.

Requirements

  • BSN in nursing from an accredited college or university required with 2 years clinical experience- or higher.
  • RN licensure in the State of NC or hold a Temporary License as a Registered Nurse in NC
  • Evidence of a high level of skill in critical thinking, assessment, identification of problems, care planning, utilization.
  • For School Health Care Manager, national certification with the National Board for Certification of School Nurses (NBCSN) is required within three years of the date of employment.
  • For School Health Care Manager, CPR required.
  • For others, CPR certification may be required depending on work location
