This position offers a $2,000 (1 year stay-on agreement) or 4,000 (2 year stay-on agreement) Sign On Bonus with the agency! This bonus will be provided upon completion of a probationary period. Responsibilities: Under the supervision of the Assertive Community Treatment (ACT) Coordinator, providing integrated behavioral and physical healthcare, including managing medication, assessing and coordinating physical/medical care, and providing direct services within the ACT Model. May be responsible for the orderly administration and maintenance of medication which involves administering prescribed drugs. Duties include a variety of supportive services and direct interventions with consumers in accordance with the community-based approach to comprehensive assertive team treatment and support for adults with serious mental illness. May render general nursing care and provide medical support in accordance with approved nursing practices. Additional responsibilities may include, but are not limited to: providing information and assistance specific to the area of assignment performing admissions screening on new consumers, serving as agency liaison, performing crisis intervention activities, providing case management, providing patient education, and providing above services on an outreach basis an indicated. Completes hospital prescreens for clients in crisis-needing stabilization. Participates and provides coverage for the ACT On-Call 24/7 System on a rotating basis. Responsible for carrying out all activities of the program/sub-unit in such a manner that fulfills CMHA-CEI’s mission, policies and procedures.
