Under the supervision of the Assertive Community Treatment (ACT) Coordinator, serves as a master’s level professional clinician performing a wide range of direct and indirect services for adults with mental health and co-occurring substance abuse disorders, along with a history of psychiatric hospitalizations. Services are primarily community based utilizing the ACT/IDDT model. Duties include therapy, case management, coordination of care regarding physical health, on-call rotation, pre-screen for hospitalization, referral recommendations to various services, consultation with service providers. Responsible for extensive case documentation. Responsible for carrying out all activities of the program/sub-unit in such a manner that fulfills CMHA-CEI’s mission, policies and procedures.
