The LPN/RN Care Coordinator performs clinical specialty care coordination to our patients, providing health advice in person and over the phone through compassion, empathy, and a strong desire to make a difference. This includes engaging in direct patient care, in all phases of their clinical office visit experience, as needed. They support the daily operations of patient flow in the office, and act as a vital resource for peers at all levels in an effort to minimize delays in care and improve operational inefficiencies. An integral part of our care team, the Nurse Care Coordinator is a trusted liaison for our patients, our providers, and our facility partners ensuring each patient feels comfortable and informed about the next steps in their care. Our team of 13 Providers deliver high quality care in a fast paced environment, so our people need to be agile and adaptable to the growing needs of our practice to deliver the same. Salary Range dependent on skills, experience, & certifications/licenses, ranging from $28 - $34 hourly.

Requirements

  • Warm, engaging, optimistic attitude
  • Willingness to learn, collaborate and grow as a human
  • Passionate about communicating with and advocating for patients
  • Graduate of an accredited School of Nursing
  • Current Pennsylvania LPN or RN License
  • BLS Certification

Responsibilities
