About The Position

Advanced Practice Provider (NP or PA) - Jersey City In-Person Role | Hospital & Post-Acute Settings | NJ License Required Seeking a compassionate and skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing team. This is an In-person position serving patients in Essex, Hudson, Union, Middlesex, Somerset, Monmouth, and Ocean counties.

Requirements

Active and unrestricted New Jersey Nurse Practitioner (NP) or Physician Assistant (PA) license (required)
Experience in palliative care, geriatrics, internal medicine, or related field preferred
Strong communication, interpersonal, and critical thinking skills
Commitment to patient-centered, compassionate care

Nice To Haves

Experience in palliative care, geriatrics, internal medicine, or related field preferred

Responsibilities

Deliver comprehensive palliative care in hospital and post-acute care environments
Conduct patient assessments, develop care plans, and manage ongoing treatment
Provide education and support to patients and families regarding goals of care
Collaborate with physicians, facility staff, and community partners to ensure continuity of care
Document clinical encounters in compliance with regulatory and organizational standards
