About The Position

The MPW Initial Assessor/ Waiver Case Manager is responsible for conducting initial MPW Assessments per State Contract for our seven-county region. MPW Initial Assessor/Waiver CM also facilitates coordination, communication, and collaboration on behalf of waiver participants to achieve goals and maximize outcomes for each participant. Also responsible for monitoring the delivery of services to the participant at various service sites.

Requirements

  • BS/BA degree in Psychology, Sociology or Social Work or other Human Services degree approved by DDID.
  • One (1) year experience in working with individuals with intellectual and/or developmental disabilities.
  • Valid driver’s license and the ability to provide safe and reliable transportation.
  • Must possess great organizational skills, time management skills, be able to read and interpret regulations, and have good communication skills (both internally and externally).
  • Physical ability to do light lifting.
  • PASRR Specialized Services CM/Waiver CM is required to complete and pass the Safe Crisis Management 2-day class prior to independent functioning. The SCM training must be re-certified annually.
  • Complete Case Management Training (conducted by DDID), core training, CPR/First Aid, and Medication Administration/seizure training before beginning independent functioning as a Case Manager.

Responsibilities

  • Ensures contact is made with participant/guardian within 7 days of notification of monthly MPW Allocation listing and an access and referral note is completed documenting discussion and scheduling of initial assessment.
  • Performs the MPW Assessment using the MAP 351 with participant & guardian and completes a per contact note. The contact note verifies staff have discussed PDS, Traditional and Blended services. It also contains information about a participant’s/guardian’s right to appeal if denied MPW services.
  • Uploads initial assessment and supporting diagnosis documentation to MWMA system for medical eligibility determinations and completes diagnosis screen.
  • Once medical eligibility (LOC) has been obtained, for the Initial MPW Assessment, Assessor follows up with individuals/guardians to let them know the status of the determination. If the individual is denied medical eligibility, MPW Assessor informs the individual/ guardian of the process to file for a reconsideration or appeal the decision.
  • Staff work with the CM agency selected by the participant/guardian to ensure all information has been obtained so services may start as quickly as possible.
  • Provides follow up to SGF Liaison and on the CHFS SharePoint about status of those on the MPW Allocation listing. (When contacted, when assessment scheduled, when completed, determination, etc.) This is done no later than the 15th of the next month.
  • Analyzes client/ guardian situations and compiles and interprets data to complete a comprehensive assessment/Individual Narrative of psycho-social needs for the client/ guardian.
  • Performs face to face contacts and needed consultations with other Service Providers/ Professionals who are involved with the overall treatment plan of the client. Contacts and consultations are performed during work hours set by supervisor based upon client need.
  • Through analysis of the data, client/ guardian contacts and consultations the case manager determines if all identified treatment needs are being addressed, identifies unmet needs, and provides intervention on behalf of the client/ guardian.
  • Facilitates team meetings in which treatment plans/ service plans are developed, modified, and reviewed. Development of plans will be based on the client’s self-assessment and will be person-centered.
  • Advocates for the client/ families interest concerning services, funding sources, treatment/ program alternatives, community resource activities, and life choices.
  • Promotes the protection of client/ guardian rights and ensures due process in the event of violations/ restrictions of rights.
  • Educates, promotes, and encourages clients in area of self-advocacy whenever possible.
  • Completes per contact notes and/or monthly summaries for each client. Information is based on the review and analysis of all services monthly data and information. Analysis of data is related back to client’s outcomes.
  • Identifies and seeks appropriate resources for resolution during a crisis. Assists individual/ family to access resources to attempt to prevent future crisis.
  • Trains direct support staff to provide needed individualized supports for each client at various service sites

  • Fosters collaborative working relationships among members of the leadership and management teams and programs
