The Mobility LPN reports to the Divisional Director of Clinical Excellence and operates under the supervision of a Registered Nurse. This role involves performing a variety of skilled activities aimed at supporting patient mobility. These activities include, but are not limited to, assisting patients with ambulation, facilitating transfers to and from chairs, guiding patients through exercises or functional activities, and performing total lifts to chairs using specialized lift equipment to achieve desired patient outcomes. A key responsibility of the LPN is to accurately update the highest level of mobility achieved by assigned patients during each shift, as well as maintaining comprehensive mobility documentation, which includes updating AM-PAC scores.
