About The Position

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of an RN Case Manager. Including, but not limited to, following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Hours / Schedule: PRN; as needed

Requirements

  • Minimum of one (1) year of working in a healthcare setting.
  • Current LPN/LVN license in the assigned state.
  • Current CPR certification.
  • Valid driver’s license with an insured and well-maintained automobile per state and organizational requirements.
  • Strong understanding of the hospice philosophy and the unique needs of terminally ill patients.
  • Adherence to professional nursing standards and best practices.
  • Excellent verbal and written communication skills.
  • Strong organizational and time-management skills.
  • Ability to work collaboratively within an interdisciplinary team.

Nice To Haves

  • Diploma or degree in practical or vocational nursing or a related field from an accredited college or university (or international equivalent) preferred.
  • Community health, hospice, or medical/surgical experience preferred

Responsibilities

  • Provide direct patient care in accordance with the State Nurse Practice Act, including specialized hospice-related duties.
  • Implement nursing interventions based on a comprehensive assessment and the plan of care.
  • Accurately document patient services and progress in a timely manner.
  • Educate patients and families/caregivers on diagnosis, care plan, and symptom management.
  • Collaborate with the interdisciplinary team to ensure continuity of care.
  • Properly use and maintain medical equipment and supplies, adhering to aseptic techniques.
  • Provide effective pain and symptom management.
  • Evaluate patient responses to treatments and medications, adjusting as needed.
  • Engage in professional growth and development activities.
  • Participate in quality assessment and performance improvement initiatives.
  • Perform other LPN/LVN duties as assigned to support department goals and initiatives.
