Requirements

• The Credentials: You hold a current LSW license and have a solid foundation in social work principles.
• The Specialist: You know your way around an MDS and understand the nuances of the PHQ-9.
• The Strategist: You don’t just check boxes; you look for ways to make processes better, faster, and more patient-centric.
• The Connector: You can translate complex clinical needs into clear plans for families and staff alike

Responsibilities

• Master the Assessments: You’ll lead the charge on MDS and PHQ-9 assessments, ensuring our clinical data is as accurate as it is insightful.
• Curate the Journey: You’ll own Discharge Planning, building seamless transitions that set our patients up for long-term success.
• Creative Evaluation: Conduct Recreational Assessments to ensure we’re treating the person, not just the diagnosis.
• Drive Best Practices: Don't just follow the manual—help us improve it. You’ll assist the facility in identifying and implementing industry best practices.
• Advise & Recommend: Act as a consultant to the interdisciplinary team, offering expert recommendations to elevate our standard of care.
• Be a Team Player: Tackle "other duties as assigned" with a proactive, collaborative spirit.
