About The Position

As a Mobile Case Manager with Chartiers Center, you would be working in the community to provide support and services to individuals with mental health challenges.

Requirements

  • Bachelor's degree in human services and 2 years of mental health experience OR
  • Master’s degree in human services with one year of experience
  • Valid Driver’s License Required

Responsibilities

  • Coordinate and oversee the delivery of case management services to ensure that the individuals are connected to various benefits and services such as SNAP, CAP, LIHEAP, LIHWAP, SOAR, OP, and MPR.
  • Assess individual’s needs and develop individualized service plans to meet those needs.
  • Maintain regular communication with clients to monitor progress and address any concerns.
  • Collaborate with other service providers and/or administrative staff to ensure seamless delivery of services.
  • Maintain strong community relationships with contacts for education, employment, housing and natural supports.
  • Maintain accurate records of interactions, service plans, and progress reports.
