About The Position

The Care Coordinator provides administrative and technical support to the Care Coordinator Supervisor and interdisciplinary team, including the members. This role focuses on the operational aspects of care coordination, including telephonic communication and coordination of services, authorization processing, service coordination, and ongoing communication with Members and care partners. This position is onsite at a Nascentia Health office, MondayFriday, 8:00 AM4:30 PM.

Requirements

  • High school diploma required; Associates degree in a related field preferred
  • Two or more years of experience in healthcare or completion of a Medical Assistant program
  • Strong computer literacy with the ability and interest to learn new systems and technologies
  • Proficiency in Microsoft Office, internet-based tools, and healthcare software applications
  • Strong analytical and problem-solving skills
  • Ability to work independently while collaborating effectively with a team
  • Excellent verbal and written communication skills
  • Demonstrated discretion in handling confidential and sensitive information
  • Ability to adapt to changing priorities, deadlines, and workflows

Responsibilities

  • Support efficient, accurate care coordination through strong organizational and communication skills.
  • Review, process, and enter authorization data accurately into computerized systems
  • Monitor and manage expiring authorization reports to ensure continuity of services
  • Assist Care Managers with scheduling and coordinating Member services as directed
  • Collaborate regularly with clinical team members and participate in team meetings
  • Act as a resource to Care Managers, supporting scheduling and tracking of Member contacts, visits, and UAS assessments
  • Answer and triage phone calls, respond to questions, and escalate issues as appropriate
  • Communicate with Members on a regular basis to support care coordination needs
  • Perform clerical and administrative tasks including filing, copying, faxing, typing, and message-taking
  • Participate in performance improvement and continuous quality improvement (CQI) initiatives
  • Perform other duties as assigned
