About The Position

Work six 24 hour shifts every 28 days and be considered full time. Each month you will have more than 7 days off in a row. This position allows you to have a great work life balance. The St. Louis County Department of Public Health seeks to hire an engaged, energetic primary care nurse practitioner or physician assistant licensed to serve adults, and children at the St. Louis County jail and juvenile detention center. This position provides outpatient, urgent, emergent and infirmary care for a diverse patient population with the support of a full time, inhouse medical director. When fully staffed, an APP is present in the facility 24/7/365 to support the medical needs of our patient population. Requirements

  • Requires Physician Assistant or Nurse Practitioner degree with Pass/Completion of the National Commission on Certification of Physical Assistants (NCCPA) or American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners Certification (AANPCB) Boards and three years of experience as an Advanced Practice Provider.
  • Meets local/national standards in care delivery, consistent with education and training, maintains all required licensures/registrations/certifications/trainings including HIPAA and any others required by DPH
  • National alignment with chronic care standards of care
  • National alignment with preventative care standards of care

Responsibilities

  • Clinical documentation/care delivery tasks (refills, clinical data review) - proficient in use of the EMR, accurate documentation that meets local/national standards, timely completion per DPH policy
  • Clinical leadership/collaborative practice - leads care team in patient-centered focus, sets expectations for staff, works well with interdisciplinary team, cultural leader, effective and timely collaborative practice work if applicable
  • Clinical productivity - completes care visits as scheduled, maintains schedule timeliness, manages appropriate panel size for FTE, patients seen per hour and show rate consistent with providers across the practice, appropriate use of telehealth
  • System improvement - supports practice and care team through QI, practice improvement, projects, accreditation, presentations, education, other assignments or duties as assigned
  • Perform diagnostic, health assessment, and patient care management duties.
  • Review patient charts and follow up.
  • Provide education to patients.
  • Work under the direct supervision of a physician.
