About The Position

The primary role of this position is to provide one-on-one brief intervention to high school students who present with risk factors to substance misuse to address use and co-occurring mental health needs. The Student Assistance Program (SAP) Mentor will provide intervention with the Project Amp model and conduct the bulk of programming, client services, and connection to supportive services and resources in the community. Additionally, this role will assist the SAP Clinician in facilitation of psycho-educational and/or diversion groups and will complete all necessary outcome tools and collect data as needed to comply with expectations of program contract through BSAS.

Requirements

Bachelors’ degree or lived experience, with a minimum of two years professional experience working with adolescents.
Ability to work with culturally diverse populations
Willingness to become trained in the Project Amp model as required by the role.

Nice To Haves

Preferred some knowledge and experience with substance use and co-occurring treatment or services for adolescents.

Responsibilities

Providing case management – conduct case management meetings regularly with parents/caregivers, key personnel from the school, law enforcement, other stakeholders and the youth, as appropriate, to discuss progress toward client-centered goals and to modify individual service plans as needed
Assisting with intake interviews, needs assessments, and individual service planning.
Working non-traditional hours as needed in an effort to accommodate youth and their families (e.g. evenings, weekends, etc.)
Providing advocacy on behalf of the youth and their family
Identifying resources in partnership with youth, establishing a referral system among community agencies and resources, and supporting youth and families in accessing these resources
Identifying and working with a family’s organic social supports, including faith-based organizations, neighborhood associations, tenant groups, community centers and clubs, etc.
Attendance at minimum weekly 1:1 supervision and other supervision/meetings as requested
Checks agency email daily and available by telephone during working hours daily
Keeps essential program tracking logs up to date
Submits required paperwork as scheduled and requested
Maintain proper and complete documentation of meetings, releases, communication, referrals and other paperwork
Regularly submits and tracks outcomes measurements
Conducts satisfaction surveys as needed
Serves as a representative for the division, program, and stakeholders through the following venues as assigned; community and school health fairs, focus groups, coalitions, and other special initiatives to increase advocacy for attention to youth substance abuse and/or promote the program and stakeholders.
Meet standards as established by performance specifications for the High School Co-Occurring Teams Grant.
