The Mental Health Rehabilitation Worker (MHRW) is an integral part of the rehabilitation of clients at Sunol Hills at Fremont. The primary responsibilities include provide support and counseling services to adolescent clients at Sunol Hills at Fremont. This is a great position for someone looking to gain valuable experience in the mental health field! We are looking for dependable, motivated individuals to join our team. We currently have openings for a full-time Mental Health Rehabilitation Workers. SCHEDULE: On-call, must pick up at least 2 shifts per month, or Monday, Wednesday and Friday 8-4pm Key Tasks and Responsibilities In collaboration with the Program Administrator and clinical team, Mental Health Rehabilitation Workers will: Coordinate and monitor treatment plans for the clients, which can include intake, assessment and discharge planning. Assist with activities involving rehabilitation and therapy. Be responsible for administrative tasks, such as completing progress notes and other client documentation. Supervise and escort clients to medical and dental appointments, community outings, and other activities. Assist with household chores, shopping and meal preparation. Complete intake and discharge process for all clients. Assist with passing and managing medications. Attend monthly staff meetings including in-service trainings. This is a non-exempt position. You will be required to work weekends and holidays. Our Crisis Residential Treatment (CRT) Program is designed for adolescents experiencing an acute behavioral health crisis. Unlike programs that are held in a psychiatric unit or hospital-like facility, Sunol Hills at Fremont is held in a natural home setting to better engage the individual in the recovery process. Our clients participate in a highly structured recovery program and work with a team of specialists who help them resolve the behavioral or emotional issues associated with their crisis and give them the tools necessary to help them work through future challenges and re-integrate back into the community. Please be advised that Sunol Hills at Fremont is taking every precaution to keep both clients and staff safe amid the COVID-19 pandemic. Incoming clients must receive a negative COVID test prior to admission, and staff are provided masks to wear at all times while onsite. We also maintain a thorough cleaning and sanitization schedule, conduct temperature checks, and currently limit visitors to solely parent/guardian meetings outdoors with clients.
