About The Position

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Our goal is to deliver quality home health services, in good faith, to those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion. The psychiatric home care nurse offers collaborative, comprehensive psychiatric nursing services and medical nursing care to patients in their home. These services are designed to prevent hospitalizations, increase access to mental health services, and provide follow-up treatment and to assist families in maintaining community living and improving their quality of life.

Requirements

  • Registered professional nurse with a current valid Pennsylvania license. and has special training and/or experience beyond the standard curriculum required for a registered nurse
  • Registered Nurse with Master’s degree in psychiatric or mental health nursing
  • Nursing experience within the last 3 years in an acute treatment unit in a psychiatric hospital, psychiatric home care, a psychiatric partial hospitalization program or another
  • Registered Nurse with a Bachelor’s degree in nursing
  • Nursing experience within the last 3 years, in an acute treatment unit is an psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other out-patient psychiatric service
  • Registered Nurse with a diploma or an Associate degree in nursing
  • Two years of recent nursing experience within the last 3 years in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or another out patient psychiatric service

Responsibilities

  • Makes initial assessment visit utilizing observation and assessment skills
  • Utilizes psycho-educational approaches with the patient and family directed toward increasing the patient’s level of knowledge and control over his/her illness, and enhancing coping skills through:
  • Symptoms and Illness management
  • Medication teaching
  • Coping Skills
  • Communication Skills
  • Manages the patient’s medical illness
  • Medication Administration monitoring and management
  • Venipuncture and monitoring of blood levels of psychotropic medication
  • Provides post hospitalization follow-up to assist in bridging the patient’s transition from hospital to community
  • Provides therapeutic interventions including:
  • Psychosocial
  • Supportive Therapy
  • Counseling – Individual and family
  • Relaxation techniques
  • Life review
  • Clinical case management
  • Performs psychotherapy interventions including:
  • Cognitive
  • Behavioral
  • Dynamic
  • Brief therapy
  • Family therapy
  • Crisis intervention
  • Caregiver support and education
  • Evaluates, instructs and reviews medications and compliance
  • Administers IM or IV medications as ordered by the physician
  • Manages situational or other crises; performs assessments of potential self harm or harm to others, and refers to the treating physicians as necessary
  • Instructs self care, mental and physical well-being, promotes independence and patient’s rights
  • Promotes and encourages patient/caregiver to maintain a therapeutic environment
  • Provides supportive counseling, psychotherapy and psychotherapeutic interventions according to education and licensure
  • Provides psycho education such as teaching/training with disease process, symptom and safety management, coping skills and problem solving
  • Provides evaluation and management of the patient’s care plan
  • Develops home health aide care plan and supervises aide care at least every fourteen [14] days in order to re-evaluate the patient’s continuing personal care needs
  • Evaluates patient care and progress at least every sixty [60] days
