About The Position

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. The Certified Physical Therapy Assistant [PTA] is a professional qualified to provide physical therapy treatment under the supervision of a Registered and Licensed Physical Therapist (LPT) to patients in the home as prescribed by the attending physician.

Requirements

  • Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association
  • Certified as an physical therapy assistant with an indirect license in the Commonwealth of Pennsylvania
  • Has performed the minimum of 2,000 hours of physical-therapy related acts which have been signed off by a supervising LPT.
  • The individual must have worked three of the five previous five years in therapy with notarization of application and copy of their resume.
  • Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a car with insurance limits as established in personnel policies

Responsibilities

  • Makes home visits to patients in accordance with the physician’s plan of care
  • Implements the therapy program established by the LPT, providing services to prevent disability, relieve pain, restoring function and independence to achieve the patient’s full potential following disease or injury.
  • Assists in teaching patients and in planning discharge from physical therapy services
  • Completes required documentation for ongoing service on the day of the visit.
  • Confers with supervising physical therapist, at least, every month, more often as patient needs dictate
  • Calls the Clinical Supervisor with any immediate patient clinical needs
  • Assists with coordinating a licensed physical therapy supervisory visit once every 7th visit or once every 14 calendar days, whichever comes first.
  • Maintains confidentiality of client information
  • Submits a schedule of patients as requested.
  • Reports patient responses to treatment, changes in care needs, progress towards goals to the supervising LPT.
