About The Position

The Florida Department of Children and Families (DCF) is the state of Florida’s social services agency. The agency oversees services for child safety, fostering, adoption, domestic violence, adult protective services, refugees, homelessness, mental health, substance abuse, childcare providers, human trafficking and public assistance. Within DCF, the Substance Abuse and Mental Health (SAMH) Program office is recognized as the single state authority for substance abuse and mental health services. The Office of SAMH is statutorily responsible for the planning, evaluation, and implementation of a comprehensive statewide system of care for the prevention, treatment, and recovery of children and adults with serious mental illnesses or substance use disorders. Northeast Florida State Hospital (NEFSH) is a 613-bed, licensed mental health treatment facility that provides person-centered services to adults suffering from severe and persistent mental illness. The hospital, located in Macclenny, was established in 1959 and is the largest state operated civil treatment facility. Requirements

  • Knowledge of rehabilitation or mental health treatment techniques.
  • Knowledge of methods of compiling, organizing and analyzing data.
  • Ability to administer or participate in major mental health service programs and functions.
  • Ability to conduct on-site evaluations of facilities and programs.
  • Ability to interpret and coordinate the implementation of mental health service programs, goals and administrative procedures.
  • Ability to maintain liaison with other agencies.
  • Ability to plan, organize and coordinate work assignments.
  • Ability to communicate effectively.
  • Ability to establish and maintain effective working relationships with others.
  • A bachelor's degree from an accredited college or university and three years of professional experience in mental health or rehabilitative services.
  • A master's degree from an accredited college or university can substitute for one year of the required experience.
  • Professional experience as described above can substitute on a year-for-year basis for the required college education.

Responsibilities

  • Coordinates the development of individualized Service Implementation Plans (SIP) for persons residing in a state mental health facility, including admission and annual SIPs. Prepares and publishes SIPs in accordance with prescribed timeframes and with prescribed content.  Chairs service team meetings, summarizes relevant meeting discussions, and serves as liaison among all disciplines involved with service provision.  Ensures that plans are developed and meetings are held in accordance with the published quarterly schedules.  Documents inclusion of resident in all planning and meeting activities.
  • Requests, receives, and reviews all necessary assessments and comprehensive evaluations from service providers to ensure completeness, accuracy, and timeliness, to be used in preparing service plans or revisions to same.  Collects and submits data on assessment completion to appropriate managers.
  • Conducts scheduled reviews of SIP progress, both monthly and annually, and updates SIP forms as required.  Completes and publishes Service Team Meeting Summaries for all service review meetings held according to prescribed timeframes.  Ensures that meetings are conducted within the scheduled timeframes.  Ensures that meetings are not cancelled and re-scheduled, without approval of supervisor.
  • Reviews, monitors, and evaluates service provision data and records to determine individual's progress toward meeting plan goals and objectives.  Makes recommendations to service providers regarding additions, deletions, or modifications to the services provided.  Documents all changes made in SIP.
  • Conducts and publishes PASIGs on all assigned residents to assess personal interests, strengths, and life goals.  Reviews findings with residents, and documents same.  Uses PASIG information in service plan development, especially in the selection of treatment/service venues.
  • Conducts discharge planning meetings as needed. Ensures that all relevant parties are in attendance and participate in meeting.  Assist with pre-discharge activities and escort /transport individuals to community site visits that facilitate discharge.
  • Participates in quality improvement activities as assigned.
  • Performs other duties as assigned.
