The Mental Health Navigator position is part of the Emergency Department and plays a critical role in supporting patients experiencing behavioral health needs during emergency visits. This position plays a vital role in supporting patients in the Emergency Department by helping coordinate timely behavioral health assessments, resources, referrals, and safe discharge planning. The Mental Health Navigator works closely with ED providers, nursing staff, and community partners to ensure patients receive the right level of support at the right time. This role serves patients of all ages, and candidates should have strong pediatric behavioral health skills, along with the ability to remain calm, professional, and compassionate in fast-paced, high-pressure situations. This is a full-time position working approximately 80 hours per two-week pay period. The primary assigned shift is 8:00 a.m. – 5:00 p.m., Monday through Friday, with an after-hours on-call rotation from 5:00 p.m. – 8:00 a.m. This role also includes weekend on-call coverage from Friday at 5:00 p.m. through Monday at 8:00 a.m., as well as holiday on-call responsibilities. Qualifications Master’s degree in Psychology, Social Work (LISW), or Licensed Professional Counselor/Clinical Resident (LPCC) required Appropriate State of Ohio credentials and licensing required EMDR experience preferred Experience working with patients of all ages, including strong pediatric behavioral health skills Strong ability to maintain confidentiality and professional boundaries Tact and diplomacy when working with individuals under stressful conditions Ability to make decisions based on urgency and patient needs Strong communication, documentation, and interpersonal skills Ability to work independently with minimal direction Commitment to teamwork, patient advocacy, and excellent customer service Key Responsibilities Provide behavioral health support and navigation services for ED patients presenting with mental health and substance use concerns Assist with screening, assessment coordination, and crisis intervention support as needed Collaborate with ED providers, nursing, and interdisciplinary staff to support safe and appropriate care plans Coordinate referrals to inpatient, outpatient, and community-based behavioral health resources Support discharge planning by ensuring follow-up services and resources are in place Communicate with patients and families in a compassionate, professional manner Maintain thorough, accurate, and confidential documentation in accordance with policy and regulatory requirements Serve as a liaison between patients, ED staff, behavioral health providers, and external agencies to promote continuity of care Promote patient safety, advocacy, and timely access to appropriate services in a fast-paced emergency environment Wyandot Memorial Hospital is a proud, independent community hospital serving Upper Sandusky and the surrounding region since 1950.
