At High Focus Centers in Farmington we are currently seeking a Counselor to join our amazing team of behavioral healthcare professionals providing Partial Hospitalization (PHP) and Intensive Outpatient (IOP) treatment services to our clients. Requirements
- LSW/LAC/LCSW/LPC (or are in the process of obtaining)
- Master's degree in a human services or related field
Responsibilities
- Provide elite-level care to adolescents seeking treatment by evaluating behavioral, emotional, cognitive, spiritual, and social needs
- Provide personalized treatment plans specific to individual needs
- Facilitate all clinical services for clients including individual and family sessions, therapeutic groups, and referral source/ outpatient provider contact.
- Provide detailed and personalized aftercare planning and community outreach resources/referrals
- Other duties as assigned
