Mental Health Case Manager Higher Ground – St Paul The mission of Catholic Charities is to serve those most in need and to advocate for justice in the community. As the need for our services grows and becomes increasingly complex, Catholic Charities is working to adapt and respond. We respond in three ways – preventing poverty, meeting basic needs in times of crisis, and creating pathways out of poverty. The Mental Health Case Manager will be embedded in one of Catholic Charities’ permanent supportive housing program and will be assigned to manage a case load of clients with mental health needs. The Mental Health Case Manager will report to the Program Manager with oversight from the Director of Behavioral Health. The practitioner will provide services that are rehabilitative and enable the client to develop and enhance psychiatric stability, personal and emotional adjustment, social competencies, community skills and independent living skills. The practitioner will support the client in strengthening skill areas needed according to their treatment plan. Catholic Charities provides services to clients in Hennepin or Ramsey County.
