Requirements

  • Bachelor’s degree with at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or a related social science; OR
  • Associate degree with at least 12 college credits in a relevant field AND two years of related experience.
  • Valid driver’s license and automobile required.
  • PA Child Abuse, PA Criminal History, and FBI clearances are mandatory.

Responsibilities

  • Inspire and motivate individuals to overcome barriers to recovery.
  • Conduct strengths/needs assessments and create individualized Service Plans based on client preferences.
  • Assist clients in accessing and securing necessary services using personal transportation as needed.
  • Maintain and update Service Plans regularly to reflect client progress and needs.
  • Build and sustain ongoing relationships with clients and their support systems to ensure effective implementation of treatment plans.
  • Monitor client participation and progress in services, advocating for their needs with service providers.
  • Facilitate access to community resources and support networks to promote daily living skills and self-determination.
  • Coordinate and ensure continuity of care within the human service system.
  • Respond to emergency situations, providing assessments and referrals as needed, including facilitating commitments to psychiatric facilities.
  • Maintain accurate and up-to-date case records and prepare required reports within designated time frames.
  • Adhere to internal and external case management standards, ensuring compliance with all requirements.
