About The Position
Mental Health APRN Position Summary : Provides skilled nursing services to patients struggling with substance use disorders. Responsible for ongoing assessment, evaluations, triage, harm reduction teaching, stabilization, and monitoring of clients/patients in the ambulatory community-based treatment model. Develops goal-oriented treatment plans with clients/patients; collaborates with primary care providers, integrated treatment team, and community providers; as well as support and services to patients from other settings referred to the program for stabilization and assessment. Collaborates with team members and outside referral sites and works with providers from addiction and community health settings to assist clients/patients and support their referral needs.
Requirements
- Considerable knowledge of principles and practices of psychiatric nursing, treatment modalities and concepts of programs for individuals with mental health and/or substance use disorders.
- Considerable knowledge of advanced nursing principles and treatments.
- Considerable knowledge of dynamics of behavior at individual, family and group levels.
- Knowledge of pharmacology as it relates to advanced nursing practice and treatment of addiction.
- Considerable interpersonal skills.
- Considerable oral and written communication skills.
- Experienced clinical assessment and evaluation skills.
- Ability to examine, diagnose, assess and treat patients for addiction and mental health issues.
- Ability to interpret diagnostic tests.
- Ability to prescribe appropriate medications and other treatments.
- Ability to provide clinical instruction.
- Ability to utilize computer software.
- Experience with assessment and development of an appropriate goal-oriented treatment plan to meet individual client/patient need with understanding and consideration of client/patient care goals as voiced by the client/patient, making the client/patient a significant part of their treatment and recovery.
Responsibilities
- Ongoing assessment, evaluations, triage, harm reduction teaching, stabilization, and monitoring of clients/patients in the ambulatory community-based treatment model.
- Develops goal-oriented treatment plans with clients/patients
- Collaborates with primary care providers, integrated treatment team, and community providers
- Support and services to patients from other settings referred to the program for stabilization and assessment.
- Collaborates with team members and outside referral sites and works with providers from addiction and community health settings to assist clients/patients and support their referral needs.
- Ongoing management of patients receiving medications for addiction and acts as a liaison between client and other community caregivers.
- Regular evaluation of client/patient progress, including crisis intervention as appropriate.
- Submit timely clinical documentation in an Electronic Medical Record (EMR) system.
- Coordination of services with the client, family, and all providers involved in the treatment plan.
- Maintain up-to-date education and professional development on current addiction treatment trends, including knowledge of the Internet and local resources for those with substance abuse.
- Provides information, education and support for clients/patient looking to access
- Provides follow-up care in person and via telephone when needed.
- Participates in a variety of cross-functional teams and performance improvement initiatives.
